First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Often we are able to complete treatment the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable.
- Be sure to pre-medicate with your antibiotic if required for a prosthetic joint or cardiac condition
- A completed health history form which can be completed here on our website under Patient Registration and submitted online or downloaded for printing.
- If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
IMPORTANT: A parent or guardian must accompany all patients under 18 at the consultation visit.
Please alert the office if you have a medical condition that may be of concern prior to being seen (i.e. latex allergy, artificial heart valves or joints, or history of rheumatic fever that requires pre-medication with an antibiotic prior to dental work.)
X-Rays
We always take our own digital x-rays which allows us to manipulate the image for maximum clarity and to take root length measurements. If your previous dentist has taken recent x-rays (within 6-months) we can use them to compare to and possibly see different angles.
